Job DescriptionThe Program Manager, Integrated Care cultivates and manages high-performing teams comprised of Nurses, Personal Support Workers (PSWs), and Allied Health professionals – fostering a collaborative, innovative, and learning-focused environment that aligns with Spectrum's Mission, Vision, and Values.In this role, you will ensure seamless program operations by leveraging data-driven insights to optimise service delivery, enhance quality, and achieve key outcomes. Lead a multidisciplinary team of nurses and allied health professionals to deliver high-quality integrated community care. Responsible for program design, clinical workflow optimization, and managing relationships with external health system stakeholders.

Requires a current CNO certificate as a Registered Nurse and at least 5 years of progressive nursing management experience, preferably in a community setting. A valid G driver's license and reliable vehicle are mandatory.
