HOME AT LAST (HAL) PROGRAM SUPERVISOR

Full-time position

35 hours per week, Monday to Friday, 8:30am to 4:30pm

Mid August start

The successful candidate must be available to work occasional evenings and weekends to support program operations.

Reporting to the Integrated Care Manager, Hospital Programs, the Home at Last Supervisor oversees the day-to-day operations of the Home at Last program, ensuring the delivery of high-quality, client-centered services that support safe and timely transitions from hospital to home. The Supervisor provides leadership and support to program staff; manages referrals, scheduling, staffing resources, and program capacity; and builds strong collaborative relationships with hospitals, Ontario Health at Home, and community partners to facilitate seamless care transitions. 

 

Qualifications

  • Post-secondary education in health care, social services, gerontology, or a related field.
  • Minimum 3 years supervisory experience, preferably in a health care or community services environment.
  • Strong leadership, communication, organizational, and relationship-building skills. 
  • Knowledge of hospital discharge planning and community support services. 
  • Coordinate daily program operations and service delivery.
  • Monitor program performance and support quality improvement initiatives.
  • Build and maintain positive relationships with healthcare and community partners.
  • Demonstrated ability to lead teams and manage competing priorities.
  • Ensure compliance with organizational policies, procedures, and relevant legislation.
  • Prepare reports, maintain documentation, and support program planning activities.
  • Strong coaching, mentoring, conflict resolution, and performance management skills.
  • Valid Ontario Driver's Licence and access to a reliable vehicle.
