Position Summary As an integral member of the interprofessional team, the Medical/Radiation Inpatient Unit Patient Care Coordinator (PCC) acts as a clinical and operational resource and co-leader for the interprofessional team. The PCC plays a key leadership role in supporting day-to-day unit operations, mentoring and providing direction to nursing and interprofessional team members, promoting staff development, supporting quality improvement initiatives, and ensuring high standards of safe, patient-centred care. The PCC works collaboratively with the Nurse Manager and interprofessional partners to support efficient service delivery, effective resource utilization, staff engagement, and optimal patient care outcomes.

Duties

The PCC has clinical and leadership expertise. The principal responsibilities within the inpatient unit include: Ensuring efficient day to day operations of inpatient unit. Determines daily staffing needs and supports the delegation and completion of assignments based on patient care requirements, workload, learning needs, and staff skill mix Ongoing continuous improvement and project management practices to strengthen inpatient processes. Performing financial/administrative responsibilities to ensure efficient and effective unit operation Contributing to staff development by promoting and ensuring a supportive/collaborative learning environment based on the principles of adult learning and practice standards for nurses, students and ancillary staff Provides direction, mentorship, coaching, and preceptorship to nursing staff, students, and members of the interprofessional team Performing human resources responsibilities in collaboration with the nurse manager Liaising with members of the multi-disciplinary team to ensure high standards of quality and optimal management of patient care outcomes (including data collection/reporting) Qualifications At minimum, a completion of BScN Baccalaureate in Nursing or Baccalaureate level education with Diploma in Nursing required Completion of a Master’s degree in Nursing preferred Basic Cardiac life Support (B.C.L.S.) program Three (3) years nursing experience, including experience as required by area of responsibility and experience working in a supervisory capacity Current certificate of competency (licensure) from the College of Nurses of Ontario required Demonstrates membership within a professional nursing association and/or nursing interest groups related to practice Demonstrated strong leadership and motivational skills Experience with project management and/or Lean process improvement an asset Demonstrates ability to work in a team environment and collaborates with others in assisting with the delivery of care to patients Demonstrated leadership abilities Excellent skills in recognizing, analyzing and resolving problems Excellent time management skills Excellent decision making and problem-solving skills Clinical and patient teaching skills Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards Exercise initiative and good judgement with the ability to multi-task Adapts to and implements change and facilitates its acceptance by others Maintains and protects patient confidentiality in accordance with UHN Confidentiality policy requirements Effective organizational, interpersonal, and communication skills Knowledge of applicable legislative, UHN and/or departmental policies Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand Additional Information Why join UHN?
